VOTIVE PALLIATIVE CARE

 

Votive is a Joint Commission Certified Palliative Care Provider.  Palliative care is a specialized type of care for individuals with chronic diseases or serious illnesses. Palliative care focuses on management of symptoms commonly related to your primary diagnosis.

Palliative care is for people that are still seeking aggressive therapy and continue to see their primary care doctors and specialists.

Palliative doctors, nurse practitioners, and nurses work together with your medical specialists to help support you and your symptoms during any stage of your illness.

COMMON QUESTIONS

 

How Often Will I Be Seen? -Your palliative care team will work together with you to help decide how frequently you will need visits. Most palliative care patients are seen once or twice per month, depending on your level of care.

Does Palliative Replace Primary Care? -No! Your Palliative Care Team will work WITH your primary care physician (and other specialists you may have) to help facilitate symptom management. If you or your loved one are in a nursing facility, palliative care can help provide support there as well.

What Will Palliative Care Do for Me? - Your palliative care team will help you achieve relief from pain, shortness of breath, fatigue, or difficulty sleeping. Palliative care strives to help you carry on with daily living. Palliative care can also help you to understand your condition and your choices in medical care.

Is Palliative Care the Same as Hospice?

NEED TO KNOW

-Palliative Care is NOT hospice. Palliative patients can have a prognosis of around 2 years. Think of palliative care as a bridge to hospice.

- Your primary care doctor will need to order an evaluation from Votive Palliative before we can become involved in your care.

-Medications are not covered under palliative care. Palliative care will only prescribe medications to manage your primary symptoms. Your primary care provider will continue to prescribe all your other medications.
